What Type of Health Provider can fill out an OCF-3?

The health care provider who can assist with accident benefit claims, particularly by completing medical forms like the OCF-3 (Disability Certificate), can be any of the following regulated professionals in Ontario:

1. Medical Doctor (MD) / Family Physician – A primary care doctor or specialist (such as an orthopedic surgeon, neurologist, or psychiatrist) can assess the claimant’s injuries and determine the extent of disability.

2. Chiropractor – If the injuries involve musculoskeletal issues, a chiropractor can complete the OCF-3 and provide a prognosis for recovery.

3. Physiotherapist – A physiotherapist can assess mobility issues and functional impairments resulting from the accident and provide treatment recommendations.

4. Occupational Therapist (OT) – An OT can assess daily functional limitations and recommend necessary accommodations or assistive devices.

5. Psychologist / Neuropsychologist – If the claimant suffers from psychological injuries such as PTSD, anxiety, or depression, a psychologist can assess the impact of the accident and complete the necessary forms.

6. Nurse Practitioner (NP) – NPs in Ontario can also diagnose and treat injuries and may assist in completing accident-related medical documentation.

7. Speech-Language Pathologist (SLP) – If the accident resulted in cognitive or speech-related impairments, an SLP can assess and document the condition.
