The OCF-1: Application for Accident Benefits form can be filled out by the following individuals:

The Injured Person (Claimant) – If capable, the injured person should complete and sign the form to apply for accident benefits.

A Legal Guardian or Power of Attorney – If the injured person is a minor or unable to complete the form due to incapacity, a legal guardian, attorney under a power of attorney, or another authorized representative can fill it out on their behalf.

A Family Member or Caregiver – If the injured person is unable to complete the form themselves, a family member, caregiver, or spouse may assist in completing it. However, the injured person (or their legal representative) must sign the form.

A Lawyer or Licensed Paralegal – Personal injury lawyers or legal representatives often assist clients in filling out the OCF-1 form to ensure accuracy and completeness before submission.

An Insurance Representative or Adjuster – While insurance companies provide guidance on completing the form, they do not fill it out on behalf of the claimant. However, they can answer questions and explain eligibility for benefits.

A Health Care Provider – While healthcare professionals do not complete the OCF-1, they may provide medical information required for other forms, such as the OCF-3 (Disability Certificate), which is often submitted alongside the OCF-1.
The completed OCF-1 form must be submitted to the claimant’s auto insurance provider within 30 days of the accident to avoid delays in receiving benefits.
